At Re-centre we dedicate ourselves to providing exceptional mental health support for all people in care.

Through Evolution Healthcare we are supported with a strong clinical governance structure. We facilitate regular robust reviews that gather key performance data from various sources to identify trends and improvement opportunities. Benchmarking is a feature of our quality systems.

At Re-centre we provide the opportunity for all those in our care to tell us about their experiences through Cemplicity, an online client reported experience measure tool. Everyone who receives care with us will be sent an electronic survey.

Through this feedback, Re-centre and Evolution are able to monitor and review the service and staff performance.

What is the Re-centre Client Experience Programme about?

The Re-centre Client Experience Program is a survey that aims to measure clients’ experiences of various aspects of care in our clinic. This survey may take around 5 minutes to complete.

At Re-centre, we are committed to delivering excellent service to our clients. Your feedback is a valuable way for us to understand where and how we can improve. As a result of the Client Experience Programme we hope to see improvements in our quality of services and the experiences and outcomes of our clients.

Around 3-5 days after your appointment, you will be invited, via email, to fill in a short questionnaire. If you wish to help us and would like to participate, fill in the questionnaire with your answers using the link provided in your email invitation. You may receive an email reminder the following week to complete the survey. The link to complete the survey will be open for 21 days.

Your help would be greatly appreciated, but it is not compulsory. If you do not wish to take part, do not complete the questionnaire.

Your response will remain anonymous unless you choose to provide your contact details to be contacted. We will not see any personally identifiable data with your response.

If you change your mind part-way through the survey, and no longer want to complete it, simply close the web browser. If you do decide to complete the survey, you can follow the same link and it will take you to what you last completed in the survey. Your response will only be submitted once you select the ‘submit’ button at the end.

You can contact our team responsible for collecting Client Experiences if you have any questions or concerns about the questionnaires, confidentiality or how your personal details will be held and used.
